📝 Registration and Requirements
To ensure the safety of all the children in our community and strictly comply with the regulations of the Department of Children and Family Services (DCFS) In Illinois, we've designed a simple and transparent enrollment process. Here you'll find all the required documents we'll need before your child's first day of school.
🗂️ Required by the State of Illinois
Official Physical Examination
- Certificate of Child Health Examination signed and stamped by the pediatrician.
- Valid for less than one year from the date of admission.
Vaccination Record
- A complete and up-to-date immunization record.
- Up to date on all the vaccinations required by Illinois health laws for your age group.
Birth Certificate
- A copy for the official verification of the child's identity and age.
Medical History and Emergency Consent Form
- The form we provide to you.
- Please list any allergies, medical conditions, and dietary restrictions.
- Include authorized emergency contacts.
💼 Help with your government subsidy
Families in the Assistance Program
If your family qualifies for or is already receiving state benefits (CCAP / Illinois Action for Children), we will need to include the following in the file:
- A copy of the current CCAP approval, specifying our daycare as the authorized provider.
Transfer or new application? Let us know in advance. We’ll personally help you fill out the Illinois Action for Children forms so the process goes quickly and smoothly.

Identification of Parents or Guardians
A copy of the driver's license, state ID, or passport of the individuals authorized to pick up the child.
Service Agreement Signed
Mutual agreement on the agreed-upon schedules, the daycare home’s policies, vacation time, and rules for getting along.
Registration Fee / Deposit
This payment secures and reserves your child's spot in our small group.
